Every insurance plan has its own scope, benefits and restrictions. Being fully informed will help your decision in requesting services, and minimizing uncovered expenses, for which you will ultimately be responsible.
We will request to make a copy of your insurance card on your visit and bill your insurance company for your visit. Should any test be necessary, we will provide that information to the labs. Our policy is to collect co-payment upon check-in. We accept all major credit cards, personal checks and cash.
If you have no insurance coverage, we require that payment be made upon service. Pre-payment may be required on certain elective services.
We welcome new patients. You can download and fill out these forms before your appointment. Please make sure to bring your insurance card on your visit. If you prefer to fill out forms on the day of your visit, please arrive at least 20 minutes prior to your scheduled time to do so.
